Temporary Full Time Position Starting June 2025 through June
2026POSITION SUMMARY: The Care Coordinator provides a full array of
outreach-based services for families of the CT DCF Foster Care
Program. The mission of the NEST HUB Consultation Program is to
reduce the number of displacements in foster care by providing
enhanced foster care support/consultation services & access to
prevention strategies while promoting family connections and
attachment. The objectives are to; Screen, and identify the need,
for additional support services; provide enhanced foster care
support for families and children; implement tiered-severity
consultations; identify metrics for data collection and analysis
and to develop policies and protocols .This position will represent
ABH-- and its services in a positive and professional manner and
adhere to ABH--'s best practices, guidelines, policies and
procedures as established, promoting ABH-- as a leader in
behavioral healthcare in all interactions and work.DUTIES AND
RESPONSIBILITIES: Provide full array of consultation services,
including services that assist families in obtaining basic supports
needed to improve their likelihood of successful attachment and
foster placement, while empowering every family to connect with
community resources that best meet the needs of the foster child,
including, but not limited to, the following:Outreach and
engagement attempts with families in various community settings,
including clients' home, community providers or other public
settings. Outreach efforts may include attempts to contact families
via visits, phone calls, or in writing;Service planning for
non-clinical needs (e.g., primary care provider, daycare providers,
behavioral health; etc.);Assistance with referrals or follow-up
care coordination to treatment and other community supports;Problem
solving to removing barriers to care;Assistance and advocacy in
accessing community support services;Referrals and contacts with
culturally relevant organizations and individuals such as the
following, as needed, to establish and maintain adequate community
supports: Early Intervention Providers;Health care providers;Crisis
intervention services, hospital emergency departments;Other
appropriate support and treatment services;Family members and other
natural supports;Interpreter services;Faith Communities (e.g.
churches, masjids, etc.);Provide fully array of Care Coordination
services, including services that guide the course of family
consultation through various levels of care. Including, but not
limited to the following: Assessment and referral to the
appropriate level of care;Intake appointment scheduling;Review of
family placement;Care coordination meetings with families are being
appropriately supported;DCF based team planning and notification of
the ASO regarding specific treatment recommendations and the
expected path of treatment through various levels of care;Case
coordination meetings for clients involved with multiple treatment
providers and community supports;Carry a cell phone during work
hours (8 a.m. - 5 p.m.) with the flexibility of being available for
appointment guided after hour calls;Carry a company issued laptop
and jet pack and conduct face to face sessions using the ABH
database where possible.Use of personal vehicles;Work out of a
regional office and attend regular regional and statewide meetings
of all Recovery Specialists;Abide by all State and Federal
Confidentiality Regulations;Comply and work towards goals
established by DCF and ABH--;Maintain internal communications -
notifying supervisors with all relevant family or program based
information;Document all case management activities within required
time frame and in required format - including documentation of all
family related activities into electronic database;Attend all
required staff, supervision and training meetings;Maintain hard
copy of clients' records (i.e. ROI) with appropriate documentation
and signatures;Function as a team member of a statewide team,
including providing as needed coverage for other members of the
team;Maintains confidentiality of all client protected health
information and adheres to all HIPAA related policies and
procedures;Demonstrates ethical behavior and cultural sensitivity
in all activities involving individuals of diverse
backgrounds;Performs other tasks/responsibilities as required to
support the business operations.EDUCATION AND EXPERIENCE
REQUIREMENTS:Bachelor's degree in Human Services, Early Childhood
or related field, with experience in behavioral health disorders;
ORThree years of experience providing services to DCF involved
families;Compliance with current State of Connecticut and federal
health and vaccine
requirements;KNOWLEDGE/SKILLS/ABILITIES:Knowledge of child
development and developmental milestones;Knowledge of child and
family serving community resources in Connecticut;Knowledge of
Cultural Diversity, Engagement and Ethics;Strong attention to
detail; ability to work on multiple tasks and meet
deadlines;Excellent PC skills with demonstrated experience using
Microsoft Office Package (MS Word, Excel, Power Point, Access,
Outlook); internet;Strong written and verbal skills
