We're seeking a dynamic and experienced Care Coordination Supervisor to oversee a multidisciplinary support team providing individualized navigation services to clients facing complex medical, behavioral, and socio-environmental challenges. This role involves direct oversight of service coordination and peer support staff, ensuring timely access to essential medical care, social services, and stabilization resources.

This individual will work closely with leadership to design, implement, and refine client engagement strategies - particularly for individuals considered clinically or socially high-risk. The Supervisor will guide the team in addressing unmet health-related social needs, including housing instability, food insecurity, legal involvement, and access to education or vocational pathways.

*Key Responsibilities:*
* Provide daily supervision and leadership to a team of care navigators and peer support staff.
* Monitor service delivery to ensure clients are connected to appropriate healthcare providers and are actively engaging in recommended treatment plans.
* Coordinate and improve protocols for managing critical transitions in care (e.g., discharge from hospitals, emergency interventions).
* Support high-acuity clients through short- and long-term goal planning, emphasizing whole-person care and recovery.
* Foster partnerships with external community-based providers and service systems to improve referral outcomes.
* Collaborate with program leadership to evaluate service effectiveness through internal tracking tools and outcome-based performance indicators.
* Participate in staff onboarding, training, and ongoing professional development initiatives.

*Qualifications:*
* Must hold a current New York State license as a behavioral health professional (e.g., LCSW, LMSW, LMHC, etc.).
* Minimum of three (3) years of direct client-facing experience in behavioral health, case management, or care coordination roles.
* At least one (1) year of supervisory or team lead experience within a behavioral health or social service setting.
* Familiarity with integrated care models and experience navigating complex systems (medical, mental health, housing, legal, etc.).
* Proficiency in engaging underserved or high-needs populations with a person-centered, trauma-informed approach.
* Strong organizational and interpersonal skills, with a flexible approach to problem-solving in a fast-paced service environment.
* Experience using data-driven tools to support care plan follow-through and staff accountability.
