HEALTH QUEST JOB DESCRIPTION APPRAISAL TITLE: DEPARTMENT: REPORTS TO: SUMMARY: Clinical Coordinator / Registered Nurse (RN) Cardiac Rehab Director, Cardiac Rehab Assumes responsibility and is accountable for, under the direction of the Medical Director and the Cardiac Rehab Director, processing of patient care, clinical staff orientation and education, staff competencies and quality improvement at The Center for Cardiac Rehab consistent with the philosophy, goals, policies and procedures of the department and Hospital Administration through effective utilization of resources. Collaborates with the Medical Director and the Cardiac Rehab Director in assessing, planning, implementing and evaluating the Center's activities.


Responsibility I - Assumes responsibility for supervision of nursing personnel to facilitate the coordination of patient's individual plan of care for all incoming patients to the program. 1.1 Acts as a resource person to promote problem-solving and harmonious relationships between staff, Medical Director, Cardiac Rehab Director and other departments. 1.2 Accomplishes timely completion of Competencies for all staff on the Clinical team. 1.3 Documents all pertinent patient information and files accordingly in patient's chart. 1.4 Assesses and reviews all medical, psychological, behavioral and social domains. 1.5 Develops new patient schedule for orientation and testing, when deemed necessary.

Responsibility II - Coordinates orientation of new clinical staff; provides education to maintain and upgrade staff and ensures staff has competent clinical practices. 2.1 Assumes responsibility for coordination of the department and hospital orientation of new hires. 2.2 Assesses competence in clinical knowledge, skills and technology ordinarily employed in care of patients in the department to the Medical Director and the Cardiac Rehab Director. 2.3 Assists the Cardiac Rehab Director in planning and implementing educational needs of the department for both patients and staff. 2.4 Holds quarterly in-services with all team members and instructs and reviews mock code, code cart, emergency situations, AED, defibrillator, safety, infection control, etc.

Responsibility III - Quality improvement - Assists the Cardiac Rehab Director in developing and maintaining a quality assurance program that meets acceptable established standards and reflects the goals and objectives of the Center for Cardiac Rehab program. 3.1 Assists the Cardiac Rehab Director in the development of QI indicators. 3.2 Participates in and makes recommendations based on results of QI reviews. 3.3 Identifies opportunities for improvement in patient care and assists manager/staff in developing plan. 3.4 Collaborates with members of medical and support staff for problem-solving of department and patient issues. 3.5 Maintains appropriate statistics for monthly analysis.

Responsibility IV - Fosters an environment for open communications and collaborative practice. 4.1 Communicates effectively and frequently with health care team members to share information and solve problems in a positive, result oriented manner. 4.2 Assists in informing all staff of new products and obtaining feedback from staff. 4.3 Informs the Cardiac Rehab Director promptly of significant information pertinent to the department. 4.4 Demonstrates and encourages a positive attitude in all staff members when unexpected staffing changes occur. 4.5 Communicates significant changes in patient's condition or daily schedule, in a timely and appropriate manner to all members of cardiac rehab team.

Responsibility V - Performs Exercise Tolerance Test (ETT) with the ability to evaluate and interpret data. 5.1 Demonstrates knowledge of testing procedures and equipment providing safety and accurate results. 5.2 Prepares room and equipment for testing including proper maintenance care of equipment and overall cleanliness of room prior to patient arrival. 5.3 Administers ETT or Pulmonary Test per manufacturer's specifications and under the direct supervision of a physician. 5.4 Documents results during test adhering to proper protocol. 5.5 Completes all required documentation after the conclusion of the test. 5.6 Provides all necessary documentation to the on-site physician for reading, interpretation and signature on the day of the test. 5.7 Adheres to Infection Control policies and procedures at all times.

Responsibility VI - Possesses ability to understand and react effectively to the implementation and evaluation of health care treatment by accurately assessing the patient's requirements relative to his/her age specific needs according to department policies and procedures. 6.1 Demonstrates knowledge of the principles of growth and development over the life span and possesses ability to assess data reflective of the patient's status. 6.2 Interprets appropriate information needed to identify each patient's requirements reflective to his/her age specific needs. 6.3 Provides the care needed as described in the department's policies and procedures.

Responsibility VII - Assumes full charge and control in a medical emergency / code situation. 7.1 Takes full charge and control of medical emergency and directs personnel accordingly. 7.2 Follows ACLS Algorithm during a medical emergency. 7.3 Defibrillates patient as indicated or directed by physician. 7.4 Administers IV drug therapy as directed by physician. 7.5 Assists paramedics and provides all medical information. 7.6 Documents the event and places in patient's chart.

Responsibility VIII - Utilizes material, equipment and time in a safe, therapeutic and cost effective manner. 8.1 Organizes workload to complete responsibilities within assigned shift. 8.2 Maintains in conjunction with Cardiac Rehab Director, supplies as needed for patient care. 8.3 Utilizes available equipment knowledgeably and effectively; reports malfunctions and other potentially hazardous situations as they arise to the Cardiac Rehab Director and/or appropriate department. 8.4 Maintains sterility of supplies by inspecting packaging and expiration date on an on-going basis, including Code Cart.

Responsibility IX - Maintains responsibility for providing support functions to team-members and other related duties. 9.1 Strives to make good use of time; seeks out work which needs to be accurately completed. 9.2 Attends all department meetings, mock codes and actively participates, on a regular basis. 9.3 Demonstrates a complete understanding of The Center's policies and procedures; reviews all manuals annually. 9.4 Performs other duties that may be necessary to fulfill the responsibilities of the position.

Responsibility X - Guest Relations: Demonstrates regard for the dignity and respect of all patients, family members and team as defined in the philosophy of VBMC. Displays a caring and courteous attitude and represents VBMC in a positive manner to all 10.1 persons noted above. 10.2 Maintains the confidentiality of team-members and departmental information with no infractions. 10.3 Adheres to departmental and VBMC dress code and wears a VBMC picture identification badge 100% of the time. 10.4 Demonstrates willingness to assist co-workers and/or to accept additional assignments as requested to support the department's efficiency as observed by the supervisor. 10.5 Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with people; uses tact, sensitivity and sound judgment at all times. 10.6 Refrains from discussing patient identity, condition or circumstances in public, in areas potentially overheard by others, or with others who do not have a legitimate interest in the patient. 10.7 Responds to all supervisor's requests for information and assists in a timely and courteous manner.

JOB QUALIFICATIONS Education: Accredited Nursing Degree Experience: Minimum two (2) years experience in Cardiac Care or related field.

License: Current Licensure in the State of New York ACLS Certified

PHYSICAL AND MENTAL FACTORS Physical: Moderate physical effort (lift/carry 501bs.), frequent prolonged standing / walking, occasionally lifts supplies / equipment, manual dexterity and mobility and considerable reaching, stooping, bending, kneeling and crouching.

Mental: Interrelationship: Frequent but generally routine contact with public, patients, team and physicians, etc.

ENVIRONMENTAL AND WORKING CONDITIONS Physical surroundings are pleasant and comfortable, with minimal exposure to injury or other health hazards. - Potential risk to blood borne pathogens. - Sufficient noise in exercise room to cause distraction.
