  • You will respond to symptom management needs and coordinate with our team to provide end of life support to patients and families.
  • Work with an interdisciplinary teamWhat You Will Need
    • Valid NYS RN license
    • Associate's degree in nursing
    • 2 years RN experience required- preferably in home care
    • Previous hospice experience preferred but NOT required.
    • Ability to navigate through several EMR platforms
    • Good communication skills.
    • Valid driver license in good standing and reliable vehicleCommunity Hospice provides a thorough orientation, preceptors and a supportive work environment. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
