Job Description:
HumanEdge is partnered with an amazing organization to recruit
strong Hospice RNs for a weekend day position. It is a 24 hour work
schedule.Admits patients to service for HospiceEvaluate hospice
patients as neededPerform assessment of patient and family, through
observation, interviews, and analysis of records.Analyze physical,
emotional psychosocial, environmental and spiritual needs.Begin
development of a comprehensive care plan.Completes paperwork
required by agency, government, and third-party systemsGraduation
from an accredited school of nursing; BSN preferredCT RN LicenseA
minimum of two years of nursing experienceHome Health Care,
Hospice/End of life care experience preferredCertified Hospice and
Palliative Care Nurse (CHPN) preferredINDCTALLIED#ZRCTALLIEDby
