Care Differently!Schedule:Flexible (3, 4, or 5-day work week)Full
Time (30-40 hours weekly)Day shift onlyNo weekendsFixed schedule/no
on-callKey Responsibilities:Must be able to function at a high
level with little oversight to provide nursing care, training, and
home support (as needed);Works collaboratively with Dialyze Direct
team members, skilled nursing home facility representatives and
dedicated Nephrologists;Collects and records relevant patient
health data in the course of/pre/post hemodialysis
treatment;Develops and maintains a plan of care that describes
interventions to attain expected outcomes;Implements and records
interventions to attain expected outcomes in EMR;Performs
laboratory collection, processing, and shipping procedures and
reschedules missed or inadequate laboratory collections;Perform
clerical tasks in addition to patient care (i.e., filing, inventory
management, admission documentation, creating patient charts,
etc.);Maintains a positive and respectful attitude to all patients
and colleagues;Occasional travel to other nearby locations;
andMedication administration and
documentation.Requirements:BS/AA/diploma in nursing with at least
12 months of RN experience (3 months in hemodialysis)
preferredValid RN licenseBLS certification; andAbility to lift up

ReimbursementCompensation:Salary Range: $32.00 - $55.00 per hour.
