Job Description:
Title: Bariatric & Obesity Medicine Patient Navigator -
Bariatric/Metabolic Department - Full Time, 40 hours per week, 8
hour shifts - 8:00am - 4:30pm. Summary: Patient navigation is a
patient centered healthcare service delivery model that supports
patient's movement along a continuum of medical care, from patient
outreach and coordination. Patient navigation aims to reduce health
disparities by assisting individuals and their families in
overcoming barriers to accessing and completing weight loss
screening, diagnostics and treatment. Responsibilities: 1. Serves
as an essential link between patients and all other care providers.
Facilitates appointments for consults and support services within
established service standards, including dietitians, specialists,
ancillary facilities, etc. Follows patients throughout the care
continuum, including inpatient admissions, and collaborates with
outpatient care management resources. 2. Obtains patient medical
records and assisting the RN or other team members with patient
care issues with internal ancillary departments to ensure that all
required paperwork/ consents are submitted accurately and timely;
facilitate follow up with candidates on missing items. 3. Create
and maintain a centralized tracking system that tracks all
necessary clinical documentation for all potential surgical
candidates and create reports as needed. Track timelines and
identify opportunities for continuous process improvement. 4.
Coordinates multidisciplinary patient support group meetings,
develops and prepares patient information and documents in
accordance with nationally recognized care guidelines. 5. Connect
candidates with Nuvance Engagement Programs and community resources
that as needed and provide additional information to provider
spouses/partner as needed. 6. Accountable for the results of key
performance metrics to track the success of the provider onboarding
process. 7. Maintain current knowledge of Nuvance Health employment
policies, benefits, and all credentialing processes to answer
candidate questions as needed or be able to direct candidates to
the appropriate subject matter experts. 8. Provide back up support
to other members of the navigator team as needed. 9. Fulfills all
compliance responsibilities related to the position. 10. Maintain
and Model Nuvance Health Foundational Values (Personal,
Imaginative, Agile, Connected). 11. Performs other duties as
required. Other information: Education Required: High school
diploma/GED Salary Range: $24.9200 - $46.2800 Hourly EOE, including
